Valproic acid
Indications/Uses
Acute or mixed manic episodes associated w/ bipolar disorder, w/ or w/o psychotic symptoms. Monotherapy & adjunctive therapy in adult & ped patients ≥10 yr w/ complex partial seizures occurring either of single or in association w/ other types of seizures. Prophylaxis of migraine in adult.
Dosage/Direction for Use
Mania Initially 25 mg/kg daily, may be increased as soon as possible to reach the lowest therapeutic dose. Max: 60 mg/kg daily. Epilepsy Adult & childn ≥10 yr Initially 15 mg/kg daily, increasing at 1-wk interval by 5-10 mg/kg daily until symptoms are controlled or side effects preclude further increases. Max: 60 mg/kg daily. Migraine prophylaxis Recommended starting dose: 500 mg once daily for 1 wk, then increased to 1,000 mg once daily.
Contraindications
Hypersensitivity. Hepatic disease or significant dysfunction. Known urea cycle disorders.
Special Precautions
Patients on multiple anticonvulsants. History of hepatic disease, pancreatitis, decreased food or fluid intake, excessive somnolence, organic brain disease. Pregnancy & lactation. Childn <2 yr, w/ congenital metabolic disorders or severe seizures accompanied by mental retardation.
Adverse Reactions
Nausea, vomiting, indigestion, diarrhea, abdominal cramps, anorexia; sedation, headache, nystagmus, dizziness, erythema multiforme, skin rash, weakness.
Drug Interactions
Antiepileptics, CNS depressants, aspirin, barbiturates, warfarin, dicumarol.
